Complete this form after you graduate from high school and have submitted all your volunteer hours to Student Financial Services. The Community Service Bursary will be sent directly to your college or university. To receive your bursary in November, you must complete this form by October 15 of the year you graduated Grade 12, or by October 15 of the following year if you deferred your bursary.

Personal information on this form is collected under subsection 32(2) of Prince Edward Island’s Freedom of Information and Protection of Privacy Act R.S.P.E.I. 1988, c. F-15.01 and will be used for the purpose of Application for Community Service Bursary.
