Personal Information on this form is collected under the authority of the Animal Health Act and the Premises Identification Regulations, as it relates directly to and is necessary for purposes outlined in the Act. The information is used to identify premises where prescribed classes of animals are kept. In emergency situations, such as outbreak of animal disease, natural disaster, or food safety issues, the collected information allows for the identification of premises/species within close proximity to the emergency. The ability to respond quickly to an emerging situation helps to minimize negative impacts on the animals, premises owners, and affected markets.
The information collected may be used and disclosed as necessary, for purposes outlined in Sections 2.1 and 2.2 of the Animal Health Act, to the Canadian Food Inspection Agency and the Government of Prince Edward Island, or their designated agents acting on their behalf. Disclosure of personal information is subject to the Freedom of Information and Protection of Privacy Act.
If you have more then one parcel of land that houses animals, you must fill out a separate application for each individual parcel.
