This form is to notify Health PEI Communications of any delayed openings, early closures, or other changes in hours due to a storm or severe weather. 

Submissions should be sent as early as possible, and by no later than 7:00 A.M. on the morning of a storm.

The information on this form is used to create news releases and to update information on the Health PEI Storm Centre web page.

Fields marked with an asterisk* are mandatory.

Identification
Fill in the fields below as the person submitting the form. This information will not be published.
For example: 902-555-5555. 
Impacted Health Service Information
Spell out the name in full. Do not use acronyms. If different services are impacted, please fill out a separate form for each service.
List the street addresses including the city or town of all locations impacted. Separate multiple addresses with a semi-colon. 
Time A.M./P.M.

Please use one of the following in this format:

  • Delayed opening until TIME A.M. or P.M. (optional: with a further announcement at TIME A.M. or P.M.)
  • Closing at TIME A.M. or P.M.
  • Closed all day
  • Other (provide details below)
     
For example: “Please call to reschedule your appointment,” or “All appointments will be rescheduled.” 
For example: 902-555-5555. This number will be public.

Reopening

Provide the reopening date and time, if known. 

If unknown, an updated submission will be required 30 minutes before your next announcement update.

If unknown leave blank.
