Personal information provided in this application is being collected under the authority of clause 31(c) of the Freedom of Information and Protection of Privacy Act R.S.P.E.I. 1988, Cap. F-15.01, for the purpose of PEI Geographic Naming and Renaming Program. This personal information will be used to identify the applicant and provide the notices required under the Program.
